Do You Actually Qualify?

The classic thresholds are BMI 40 or above, or 35 or above with an obesity-related condition such as type-2 diabetes, sleep apnoea, hypertension or severe joint disease. Those have guided practice for decades and still frame most insurance criteria.

Major bariatric societies now also support surgery from BMI 30 to 35 in patients with type-2 diabetes that medication is not controlling, because metabolic benefit does not track body weight alone.

What disqualifies: untreated eating disorders, active substance dependence, unmanaged severe psychiatric illness, and inability to attend follow-up. These are not moral judgements. Each one predicts a poor outcome, and each is addressed before surgery rather than instead of it.

Sleeve Or Bypass: Five Questions.

  • Do you have reflux? Significant existing reflux usually points to bypass, because a sleeve can worsen it. A sleeve in a patient with severe GERD is the commonest reason for later conversion surgery.
  • Do you have type-2 diabetes? Bypass produces higher remission rates and faster glycaemic improvement because of hormonal changes in the rerouted gut.
  • What is your BMI? Very high BMI tends to do better with bypass.
  • How do you eat? Volume eaters without reflux do very well with a sleeve. Sweet-eater patterns tend to do better with bypass.
  • Can you take supplements for life? Bypass requires lifelong B12, iron, calcium and vitamin D with annual bloods. If you know you will not take them, the sleeve is the safer operation for you, and an honest answer here prevents anaemia years later.

The Two-Week Liver Diet Before Surgery.

A fatty liver sits directly over the stomach and is heavy, friable and hard to retract safely. Two weeks of a low-carbohydrate, high-protein diet shrinks it measurably, which makes the operation shorter and safer.

It is not a test of willpower or a head start on the diet. Surgeons cancel operations on the table when the liver is too large to lift, and that is the outcome the two weeks exist to prevent.

The First Week, Honestly.

  • Day 0-1. Sips of water only, walking within hours. Walking is the single most effective thing you do for clot risk.
  • Days 2-3. Clear liquids. Nausea is common and is medicated. A leak test is performed before you are discharged onto fluids.
  • Week 1. Full liquids. Fatigue is heavier than people expect, because you are healing on a few hundred calories a day.
  • Weeks 2-4. Pureed, then soft. The phases are not negotiable; they protect a staple line that is still sealing.
Protein first, always. Sixty to eighty grams a day from the soft phase onward. Protein is what decides how much of the weight you lose is fat rather than muscle, and it is also tied to the hair shedding most patients see between months three and nine.

Weight Regain: What Actually Causes It.

The stomach does not stretch back to its old size. What returns is capacity at the margins, and more importantly the eating patterns that were there before.

The two most common mechanisms are grazing, which bypasses restriction entirely because small volumes fit at any time, and liquid calories, which pass straight through. Both are behavioural, both are predictable, and both are why the follow-up programme is part of the operation rather than an add-on.

What can I eat after the operation?

Two weeks of liquids, two weeks of purées, then soft food and gradually normal meals in small portions with protein first. Your dietitian gives you a written plan for each phase in your own language and checks in monthly for a year.

Will I have loose skin after losing weight?

Some patients do, usually 12-18 months after surgery once the weight has stabilised. If it bothers you, a tummy tuck, arm lift or body contouring can be planned then. We quote those separately when the time comes, never as an upsell on day one.
