Grafts start losing viability after about two hours out of the body. Here is what happens in those hours, and why the holding solution matters more than speed.
Follicular grafts stay reliably viable for around two hours outside the body, and survival starts to fall measurably beyond four to six. The follicle is a living organ with no blood supply once it has been extracted, so it burns through its stored energy and then depends entirely on the temperature and chemistry of the solution it sits in. Every extra hour on the table is a quiet tax on the count you see in the mirror three years later.
Chilled hypothermic storage slows follicular metabolism to a fraction of its normal rate, and that is the entire mechanism behind graft holding. We keep grafts in a hypothermic holding solution at around four degrees, in shallow chilled trays, rather than in warm gauze on a side table. Room-temperature saline is the cheap version and it is measurably worse over long sessions. Ask any clinic which solution they use and at what temperature; a vague answer means nobody is counting the hours.
A four thousand graft session means four thousand individual extractions and four thousand individual placements. If extraction runs at eight hundred grafts an hour and implantation at a similar rate, the first graft out waits several hours before it goes in. That is why we split large sessions into batches: extract a block, implant that block, then extract the next. It looks slower on paper and it keeps out-of-body time under control for exactly the grafts that would otherwise wait longest.
You cannot audit a holding solution from a waiting room, but you can ask three questions and listen to how fast they are answered: which solution the grafts sit in, at what temperature, and whether extraction and implantation run in batches. Ask also how many technicians work on your case, because two pairs of placing hands over eight hours is a very different situation from six pairs over four. If the answers stay vague, the honest advice is to keep looking.
Not automatically. A twelve-hour day split into batches with cooled holding solution can produce better survival than a rushed six-hour session in which every graft waits. What matters is how long each individual graft is out, not how long you sit in the chair.
No. We do not extract on one day and implant on the next, and we would advise against any clinic offering it. When a case genuinely needs two days, we extract and implant fresh grafts on each of them.
