Diastasis recti: what the muscle repair in a tummy tuck does

The muscles are not torn and nothing is missing — the sheet between them is stretched, and only a suture line shortens it.

What the separation actually is

The two columns of the rectus muscle are joined down the midline by a sheet of connective tissue. Pregnancy or a large weight change stretches that sheet so the muscles sit further apart. Nothing is torn and nothing is missing — the sheet is simply wider and slack. That is why the abdomen bulges forward when you sit up from lying, even in a slim patient who has otherwise returned to her pre-pregnancy weight.

Why exercise cannot close it

Training thickens the muscles but does not shorten the stretched sheet between them, and some exercises that raise pressure inside the abdomen make the bulge more obvious rather than less. For a mild separation, physiotherapy focused on the deep core is genuinely worth trying first, and we say so before offering an operation. When the gap has been present for years and measures several centimetres, no exercise programme closes it.

What the repair involves

During a tummy tuck the skin is lifted off the abdominal wall and the inner edges of the two muscle columns are drawn together with a continuous permanent suture from the ribs to the pubic bone. Surgeons call the result an internal corset, and it narrows the waist by several centimetres. It is also the part of the operation responsible for most of the early discomfort and the reason you walk slightly bent for a few days.

What it changes and what it does not

You get a flatter profile, a narrower waist and often better core mechanics. What you do not get is a hernia repair — a true hernia has a defect with contents pushing through it and is treated differently, sometimes with mesh, which is why we examine for one before planning. Nor does the repair remove fat inside the abdomen: a belly that is firm and round from within will not flatten. Tummy tuck starts at €3,600.

Lie on your back, lift your head off the pillow and press your fingers along the midline. Skin laxity feels soft and pinchable; a separation feels like a soft channel between two firm ridges that appears when you tense.

A pregnancy after the repair can stretch it again, and significant weight gain can too. We advise completing your family first, then repairing once.
