The sleeve is created with a surgical stapler that cuts and seals in the same pass, leaving a staple line roughly twenty centimetres long running down the length of the new stomach. That line is the one place the sleeve can fail in the first days, so it is tested while you are still asleep rather than assumed to be sound and checked later.
With the sleeve complete, the surgeon clamps below it and fills it through a tube with methylene blue dye or with air while the abdomen is filled with saline. Dye escaping along the staple line, or bubbles rising through the fluid, shows a defect immediately, and it is repaired on the spot while the field is open and clean. Finding it now means one extra suture; finding it on day three means a serious complication.
Some surgeons buttress the staple line with an absorbable strip or oversew it with a running suture.
