The gynecomastia scar: where it sits and how it fades
In most cases it is a 3-4 cm line along the lower rim of the areola, hidden by the colour change between dark skin and light. Here is what makes it visible.
Where the incision goes
It sits along the lower half of the areola border, following the natural colour boundary between pigmented and normal skin. That line is typically three to four centimetres long, which is enough to deliver the glandular disc through it. Liposuction is done separately through two three-millimetre entry points hidden in the fold under the chest and in the armpit. No incision crosses the visible skin of the chest itself.
The twelve-month fade
Scars get worse before they get better. Expect a raised pink line at six weeks that looks more obvious than the day the stitches came out; this is normal collagen remodelling and it peaks around month two or three. From month four the line flattens and loses colour, and by twelve months most patients have a pale mark that is hard to find in a mirror without looking for it. Sun exposure during that first year is the main thing that fixes pigment permanently.
When the scar has to be longer
Grade three chests with significant loose skin cannot be closed through the areola alone. If the skin envelope will not retract, usually after major weight loss, or when the nipple has dropped below the chest fold. We discuss a larger pattern that leaves a visible scar on the chest. We say this at the consultation, not in the recovery room. Some men choose the smaller scar and accept residual looseness; that is a legitimate trade and we plan for it openly.
What you can actually do about it
Silicone sheeting or gel applied daily from week three onwards is the only topical measure with consistent evidence behind it, and it needs three to six months of use to matter. Keep the area out of direct sun or use a high-factor block for a full year. Do not massage the wound before it has closed, do not use vitamin E creams on an open scar, and tell us early if the line thickens rather than flattens. That pattern responds to steroid injection.

At twelve months most areola-edge scars are a pale line that blends into the pigment border and is not noticeable at conversational distance. Darker skin types can develop a raised scar, which is why we ask about your family's scarring history before surgery.
How soon can I expose the scar to the sun after gynecomastia surgery?
Keep it covered or under a high-factor sunblock for the first twelve months. Ultraviolet light on an immature scar drives pigment into it, and that darkening does not reverse on its own.
