Gland has to be cut out. Fat can be suctioned. Most chests are a mixture, and getting that proportion wrong is why liposuction alone leaves a lump behind the nipple.
The Pinch Test, And What It Tells Us.

Why We Scan Before We Schedule.
A physical exam is not enough on its own. We use breast ultrasound to measure how much glandular tissue is present and to rule out anything that does not belong there, and blood tests to check testosterone, oestradiol, LH, prolactin and thyroid function.
If a hormonal cause is active and untreated, surgery removes the tissue but not the driver.In a small number of men the scan finds a lesion that needs investigating before any aesthetic plan is discussed. Some medications are also a known cause, and stopping them under medical supervision occasionally resolves the problem without an operation.
Pure Fat: The Case Where We Say No.
If the ultrasound shows no gland and your body fat is high overall, we will tell you surgery is the wrong first step. Chest fat responds to the same weight loss that shrinks the rest of you, and operating on a chest that is still changing shape produces a result you will not keep.
We ask those patients to come back after a stable period at a lower weight. It is a shorter conversation than either side wants, but it protects the outcome.
What The Diagnosis Changes About The Operation.
A mixed chest needs both: Vaser liposuction to feather the fat across the chest wall, then direct excision of the glandular disc through a small incision at the lower edge of the areola.
The scar sits along the border between areola and skin, where a colour change already exists, which is why it settles well. It is pink for two to three months and pale after that.Liposuction alone on a glandular chest leaves the disc behind, and the lump under the nipple is the most common reason men come to us for revision. Pure liposuction cases sit at the lower end; heavy skin excess costs more.
Recurrence, And The Six-Week Rule.
Gland that has been excised does not grow back. What can return is fat, if weight climbs, and gland can reappear if the hormonal driver is active and untreated, which is exactly why we test for it first.
For training: light lower-body work from week two, upper body from week four, and full chest work at six weeks. Pushing that timeline stresses a closure that is still soft and is the usual reason a scar widens.
