Loose skin after major weight loss: what helps and what doesn't

Why skin does not always retract
Elastin fibres damaged by years of stretching do not regenerate. Age, smoking, sun damage, the amount lost and how fast it was lost all determine retraction, and none of them respond to topical products.
What genuinely helps
Adequate protein during rapid loss, resistance training to fill the frame with muscle, hydration, stopping smoking and losing weight at a steady rather than extreme rate. Together these make a visible but modest difference.
When to consider surgery
Twelve to eighteen months after weight has stabilised. Operating earlier means removing skin that would still have retracted, and further weight loss afterwards loosens the result again.
What body contouring involves
Tummy tuck, arm lift, thigh lift or a lower body lift depending on the pattern. These are real operations with long scars traded for shape, a trade most post-bariatric patients consider worthwhile, but it should be an informed choice.
