Gastric botox: what the injection can and cannot do
Botulinum injected into the stomach wall slows emptying for three to four months. It is not a substitute for a balloon or a sleeve, and we say so plainly.
How the injection is supposed to work
The procedure is an endoscopy, not surgery. A gastroscope carries a fine needle into the stomach and botulinum toxin is injected into the muscle layer of the antrum, the pumping section near the outlet. The relaxed muscle empties more slowly, so an ordinary meal sits longer and satiety lasts longer. It takes twenty to thirty minutes under light sedation, leaves no incision and needs no hospital bed. The effect fades as nerve endings regenerate, usually within three to four months.
What the evidence actually shows
Results are modest and highly variable. Published series report average losses in the region of five to ten kilograms over four to six months, alongside a meaningful group of patients who lose nothing measurable at all. There is no anatomical change and no mechanical restriction, so nothing prevents the old portion sizes returning once the toxin has worn off. Gastric botox is not classified as a bariatric operation by any surgical society, and we do not present it as one.
Who it might genuinely suit
It can be reasonable for someone with a body mass index between 27 and 32 who needs a three-month push to break a plateau while a genuine eating and exercise habit is being built, or for a patient who cannot tolerate a balloon. It is the wrong tool above a body mass index of 35, in significant reflux, in a large hiatus hernia, and for anyone hoping to dodge an operation their comorbidities already justify.

Is gastric botox permanent?
No. The toxin is broken down and nerve endings regenerate, so the delayed emptying ends after roughly three to four months. Any weight kept beyond that point is kept by habit, not by the injection.
Can I have it repeated?
It can be repeated, but doing so every few months turns a cheap procedure into an expensive one and still leaves the anatomy unchanged. At that stage a balloon or a sleeve is usually the more honest option.
