How long does a facelift last, and what shortens it?

A deep-plane facelift typically holds for ten to fifteen years, but it does not stop ageing. What it resets is the position of tissue, and four habits decide how long that lasts.

The honest number

A well-performed deep-plane facelift usually holds its result for ten to fifteen years, while a skin-only tightening can loosen within three to five. The reason is where the tension sits: when the deep layer carries the load, the skin is laid back without stretch and has nothing left to stretch out of. Ageing continues from the day after surgery regardless. The useful way to think about it is that you will always look younger than you would have without the operation.

What shortens it

Four things reliably shorten a facelift result. Smoking damages the small vessels that feed the skin flap and accelerates elastin loss. Unprotected sun exposure does the same from the outside. Large weight swings after surgery stretch and then empty the tissue that was repositioned. And significant weight loss on medication or after bariatric surgery deflates the face faster than any operation can compensate for, which is why we ask patients to reach a stable weight first.

What a facelift never fixes

A facelift repositions tissue; it does not change skin quality, and this is where disappointment usually comes from. Fine lines around the mouth, sun damage, pigmentation and the texture of thin crepey skin all need resurfacing, not lifting. Neither does it treat the upper face: hooded eyelids and a heavy brow are separate operations with separate anatomy. Being told that in consultation is far more useful than discovering it in the mirror at three months.

Second facelifts, and when we say no

Some patients return after twelve or fifteen years for a smaller secondary lift, and that operation is generally easier than the first because the deep layer has already been released. What we decline is the patient who wants a second facelift at three years because the result was never what they imagined. That is a conversation about expectations, not an operating list. Our facelift is €4,800, and we would rather lose a booking than perform surgery that cannot deliver.

How long does a facelift last, and what shortens it?

There is no good evidence that they do, and scar tissue from repeated threads can make later surgery harder. Sun protection, a stable weight and not smoking have far more effect on longevity.

Done properly, they notice that you look rested rather than different. The signs that give it away — a pulled mouth corner, a displaced sideburn, a flattened ear lobe — come from technique, not from the operation itself.
