Native scalp carries 70 to 100 follicular units per cm². A transplant that reaches 40 to 45 in the hairline already reads as full, and here is the arithmetic.
Forty to forty-five follicular units per square centimetre in the hairline looks full to the naked eye, even though native scalp carries seventy to one hundred. The eye does not count hairs; it reads the contrast between hair and scalp. Once roughly half of native density is restored and the hairs leave the skin at the right angle, that contrast disappears at conversational distance. Chasing native density is both impossible with a finite donor and unnecessary for the result you are actually asking for.
Blood supply is the limit. Each channel is a small wound, and channels placed too close together compete for the same capillary bed, which is how over-dense hairlines end up with patchy survival and a visibly damaged strip. Sapphire blades cut narrower, shallower channels than round steel punches, which is what makes forty-five units per square centimetre safe rather than ambitious. The ceiling itself is biological, and no blade material removes it.
Measure the thinning area in square centimetres, multiply by your target density, and you have your graft number. A receding hairline covering fifty square centimetres at forty-five units needs about 2,250 grafts. A crown of seventy square centimetres at thirty units needs 2,100 more. Add the two and compare the total against what the donor safely allows. When the sum exceeds it, we lower crown density rather than hairline density, because the hairline is the part people look at.
Behind the hairline we deliberately drop to thirty or thirty-five units per square centimetre, and in the crown to twenty-five or thirty. Grafts placed behind a dense front row are seen through existing hair and contribute far less visible change each. Spending them there is poor value for a resource you cannot replace. If a clinic quotes uniform high density across the whole scalp, ask what will be left in the donor when you need a second session.
Not reliably. Above roughly forty-five units per square centimetre survival becomes less predictable because channels start competing for blood supply, and the visible gain is small. We would rather place the difference somewhere it still changes the picture.
Because density builds as hairs thicken, not as they appear. At month six most hairs are present but at a fraction of their final calibre, and calibre is what blocks light. Density should be judged at month twelve.
