Can you add density without losing what you have?

Yes, in most cases. The DHI implanter opens the channel and delivers the graft in a single movement, so the surgeon works inside the narrow gaps between the hairs you still own. With sapphire FUE the channel is cut first, and in a scalp that still holds sixty percent of its density that blade will transect living follicles it cannot see. That mechanical difference is the whole reason we recommend DHI for thinning areas and not for bald ones.

Who this is genuinely for

The strongest candidates are men at Norwood two or three with a receding frame but healthy hair behind it, and women with diffuse mid-scalp thinning who still have a stable donor at the nape. If the area is already smooth and shiny, the follicles are gone, there is nothing left to protect, and sapphire FUE at €1,800 does the same job for less money. We tell patients that rather than selling the more expensive technique by default.

The shaving question, answered honestly

The recipient area does not need shaving for DHI, but the donor usually does. A fully unshaven case is possible, costs more in chair time, and realistically caps the session near 2,000 to 2,500 grafts because extracting through long hair is far slower. If your plan needs 4,000 grafts and you cannot shave, the honest options are two sessions or a partial trim hidden beneath longer hair on top. Anyone promising 4,500 unshaven grafts in one day is overselling.

Cost, shock loss and what is guaranteed

DHI is €2,300 as a package. Expect shock loss between weeks two and six: some existing thinning hairs shed alongside the transplanted ones and the area can look worse before it improves. Nearly all of it returns by month five.
