The first wash after a hair transplant, step by step

Day three, and why not earlier

The first wash is done on the third day after surgery, in the clinic, by someone who performs it several times a day. Before that the grafts are held mainly by fibrin and are still easy to dislodge; by day three they are anchored well enough to tolerate lotion and a gentle flow of water. Washing earlier risks moving grafts, and washing later lets crusts harden around the shafts, which then take a fortnight to lift instead of a week.

The sequence, exactly

Apply the lotion or foam gently over the recipient area with fingertips and leave it for thirty minutes so the crusts soften. Lather the medical shampoo in your palm first, never squeezing it straight onto the grafts, and pat it on without rubbing. Rinse with lukewarm water poured by hand, or from a low-pressure shower held about thirty centimetres away, never a direct jet. Dry by pressing with a clean paper towel rather than a bath towel, and never rub.

The ten-day routine and what the crusts do

Repeat that wash once a day for ten days. Crusts lift gradually between roughly day seven and day twelve, and pulling one early can bring the graft with it, which is a lost graft rather than a lost scab. Some hairs come away with the crusts, which alarms people; the hair falls, the follicle stays. The donor area heals faster and can be washed more normally after day three, though the same rules on pressure and rubbing apply.

What sapphire channels change about this week

Narrow V-shaped sapphire channels close faster and crust less than wider round incisions, so most patients look visibly clean by day ten rather than day fourteen. That is a comfort difference, not permission to wash sooner. From day eleven you can shampoo normally with light fingertip pressure. No scrubbing, no barber basin, and no pressurised shower head aimed at the recipient area until the end of the first month.
