Cycles often restart within three months of a sleeve, while the oral pill may absorb unreliably. The eighteen-month window matters for both mother and baby.

Why cycles return so quickly

Falling insulin and androgen levels release the brake on ovulation long before the mirror shows much difference. Many women ovulate again within eight to twelve weeks of a sleeve, and ovulation can occur before the first normal period arrives, which means conception is possible while the calendar still looks irregular. Couples who spent years being told they were unlikely to conceive are the ones most often caught out, because nobody revised that sentence for them after the operation.

Why the pill stops being reliable

Absorption is the problem. Vomiting and low intake during the first weeks can flush an oral contraceptive out before it has worked, and after a bypass the stretch of small bowel that would have absorbed it is no longer in the path of food. The safer choices are routes that skip the gut altogether: a copper or hormonal intrauterine device, a subdermal implant or an injection. Arrange this with your own gynaecologist before you travel, not afterwards.

Why we ask for twelve to eighteen months

During rapid weight loss the body runs a deficit, and a pregnancy in that window competes with the mother for iron, vitamin B12, folate and protein. That period is associated with more small-for-gestational-age babies and more maternal deficiency. Once weight has stabilised the picture reverses: compared with pregnancy at the previous weight there is less gestational diabetes, less pre-eclampsia and fewer caesarean sections. Waiting is not caution for its own sake, it buys a better pregnancy.
