Where the nipple sits against the fold

The decision is made by one landmark: where your nipple sits relative to the crease under the breast. Above the crease, an implant alone will fill and lift enough. At the level of the crease, it is borderline and depends on skin quality. Below the crease, no implant lifts it — adding volume to loose skin makes a larger breast that still points downwards, which is the single most common disappointment in this operation.

What a lift costs you in scars

A lift removes skin, and skin removal always leaves a scar. A mild case may need only a scar around the areola; most need that plus a vertical line down to the fold, and heavy, long-standing sagging needs an anchor shape with a scar along the crease as well. The scars are red for three months, pink for a year and pale after that. We show photographs of scars at six weeks, not only at twelve months.

Doing both at once, and when we split them

A lift with an implant in the same operation is routine and it is what most women who have breastfed actually need: volume for the emptied upper pole, skin removal for the position. It is a longer operation with a slightly higher revision rate than either alone, because the surgeon is moving the nipple and changing the volume it has to sit on at the same time. In very heavy or very tight cases we stage it deliberately.

What we will not do

We will not put a large implant into a breast that needs a lift because the patient wants to avoid a scar. It produces a heavy, low breast that ages badly and comes back for revision within a few years, and the revision then needs the same scar plus a capsule problem to solve.
